At Strawberry Fields, this service goes beyond connecting people to resources—it's about walking with them on their recovery journey and celebrating their growth toward self-sufficiency.
Last fiscal year, we provided case management to 273 children, youth, families, and individuals. Our case managers assist with practical needs like finding housing, seeking employment, scheduling medical appointments, and connecting to therapy services. They also help clients develop daily living skills and create action plans for their goals.
The relationship between a case manager and a client is built on trust and mutual respect. "My case manager stands by me and my decisions and does not give up on me," shares one client. Our approach focuses on supporting individuals in making their own informed choices rather than prescribing solutions.
Recovery looks different for everyone. Some clients may focus on maintaining stable housing, while others work toward educational goals or building stronger community connections. As part of our holistic approach, our case managers and clients work with our dedicated mental health nurse navigator, who provides care coordination between physical and mental healthcare services, creating a more comprehensive support system leading to better outcomes.
